At Rapha Care, we are committed to enhancing the quality of life for individuals with disabilities, complex needs, and behaviours of concern. Our Positive Behaviour Support (PBS) services provide evidence-based strategies that focus on reducing challenging behaviours, increasing positive engagement, and empowering individuals to thrive in their daily lives.
Challenging behaviours often arise due to a misalignment between an individual’s social or physical environment and their ability to navigate it effectively. Our highly skilled Behaviour Support Practitioners work closely with individuals, families, carers, and support teams to identify the underlying causes of behaviours of concern and develop proactive strategies that promote lasting change.
Our ultimate goal is to empower individuals to live more fulfilling, meaningful lives while reducing the need for restrictive practices.
Our Positive Behaviour Support Services
Rapha Care offers NDIS-funded Behaviour Support under the Improved Relationships category (or Behaviour Support under a PACE plan), including:
✔ Interim Behaviour Support Plans – Rapid response for high-risk behaviours and restrictive practices while a comprehensive plan is developed.
✔ Functional Behaviour Assessments (FBA) – Identifying the root causes of behaviours and developing data-driven intervention strategies.
✔ Comprehensive Behaviour Support Plans (CBSPs) – Personalised, strategy-driven plans focusing on prevention, skill development, and overall quality of life improvements.
✔ Plan Implementation & Training – Hands-on support for families, carers, and staff to ensure effective plan execution and skill development.
How Does Positive Behaviour Support Work?
PBS is a structured and collaborative approach that helps individuals replace behaviours of concern with positive, functional alternatives. Our Behaviour Support Practitioners conduct thorough Functional Behaviour Assessments (FBA) to determine why behaviours occur and then implement Personalised Behaviour Support Plans (PBSPs) tailored to each individual’s needs.
We focus on:
- Understanding triggers and underlying causes of challenging behaviours.
- Developing proactive strategies to prevent behaviours from escalating.
- Enhancing emotional regulation, communication, and coping skills to improve social engagement.
- Providing practical training for families, carers, and support staff to maintain positive change in all settings.
- Reducing reliance on restrictive practices in favour of evidence-based interventions
Our team ensures that each Behaviour Support Plan is practical, sustainable, and focused on improving long-term well-being.
Where Do We Provide Support?
Our local Behaviour Support Practitioners provide services across various settings to ensure support is accessible, practical, and relevant to everyday life. We offer:
- In-home support – Working with individuals and families in their home environment.
- School-based support – Collaborating with teachers and staff to enhance positive engagement in education.
- Workplace & community-based support – Helping individuals navigate social interactions and independent living.
- Telehealth support – Providing remote services for increased accessibility and flexibility.
- Clinic-based sessions (where available) – In-person therapy for structured interventions.
What Does a Positive Behaviour Support Practitioner Do?
A Positive Behaviour Support (PBS) Practitioner supports children and adults with behavioural or emotional challenges by identifying the reasons behind behaviours of concern and developing strategies to promote positive change and well-being.
They work closely with individuals, families, and support teams to:
✔ Modify environments to create supportive, structured spaces.
✔ Teach new skills to improve communication and emotional regulation.
✔ Address underlying causes of behaviours, such as unmet needs or triggers.
✔ Reduce the need for restrictive interventions by using proactive, person-centred approaches.
By focusing on strengths-based, evidence-based strategies, PBS Practitioners empower individuals to navigate their world more effectively and improve their quality of life.

Key Components of Positive Behaviour Support (PBS)

Functional Behaviour Assessment
Our team conducts comprehensive assessments to identify the underlying causes of behaviours of concern. By analysing environmental factors, triggers, and past experiences, we develop a clear understanding of what influences the behaviour.

Individualised Behaviour Support Plans
Every participant receives a customised Behaviour Support Plan (BSP) designed to suit their unique needs and goals. These plans focus on teaching new skills, improving communication, and equipping individuals with positive behaviour management strategies.

Reducing Restrictive Practices
Aligned with NDIS guidelines, we are committed to minimising restrictive practices such as physical restraints or seclusion. Our practitioners implement alternative strategies that prioritise safety, dignity, and long-term positive change.

Collaboration & Support
Our approach is collaborative, involving participants, families, carers, and key stakeholders to ensure that support plans are practical and aligned with individual lifestyles and goals. This teamwork fosters an environment where individuals can thrive.

Ongoing Monitoring & Review
PBS is a continuous process, and we regularly track progress, evaluate interventions, and update Behaviour Support Plans as needs evolve. This ensures that our strategies remain effective and responsive to individual development.

Customisation of Services
Our PBS services are fully adaptable, designed to align with each participant’s NDIS goals and personal needs. We work closely with all involved parties to create flexible, individualised support solutions that drive meaningful outcomes.
What If I Don’t Have Behaviour Support Funding Yet?
If your NDIS plan does not currently include Behaviour Support funding, we can still provide services through your Improved Daily Living (IDL) funding. Our Behaviour Support Practitioners can assess whether Positive Behaviour Support (PBS) is needed and provide a recommendations report to support a funding request in your next NDIS plan review.
For participants displaying behaviours of concern who have never received a PBS assessment or intervention, the first step is to complete a Behaviour Support Needs Assessment. This assessment, which requires five hours of funding from the Improved Daily Living budget, is conducted by a PBS Practitioner to determine the level of support required.
This process does not require face-to-face visits and is typically done remotely through a phone call or online meeting (via Teams) with a family member, carer, or referrer who provides key information about the participant’s behaviours. The PBS Practitioner then prepares a concise report outlining the participant’s behaviours of concern and their impact on daily life.
