Our community nursing team of both qualified registered and enrolled nurses will work with you to comprehensively assess and design an individualised plan of care for your disability health related needs and determine the best approach based on your NDIS goals.
Such assessments and plans for ongoing requirements include:
- Medication Management
- PEG Feeding
- PEJ Feeding
- Wound Managements and Dressings
- Diabetes support
- Continence support
- SPC insertion and Catheter Care
- General nursing care – including assisting with chronic conditions
- Palliative care
- Tracheostomy care
- Administration of Injections
- Transfers to hospital
- Social Support
These supports can be delivered in our accommodation, or in the community.
